You can now fill out the New Patient Kit online. To do so, click on the Learn More button below. When the file opens, you can fill out the information. When finished, press the email button and it will be sent to Retina Associates of Florida. Be sure to print a copy for yourself or click on the save button to save an electronic file. If you'd prefer, you can still print out the form and fill it out by hand. You will need to bring the completed New Patient Kit to your first appointment..
